Hospice Physician
Oversees each patient’s plan of care, coordinates with the primary care provider, and ensures that all medical needs are managed effectively and with compassion.

Registered and Licensed Nurses
Conduct regular visits, administer medications, and provide expert symptom and disease management while offering education and emotional support to families.

Certified Nursing Assistants and Home Health Aides
Offer hands-on assistance with personal care needs such as bathing, dressing, feeding, and other daily activities, ensuring every patient’s comfort and dignity.

Interdisciplinary Care Team
A group of professionals — including social workers, chaplains or bereavement counselors, and physical therapists — who provide emotional, spiritual, and rehabilitative support to patients and their loved ones.
